I am paying for my extended medical benefits at my work while I am off on a leave. Work has sent me a T4 for 2023, but it did not include any of the medical benefits I had paid. I had to request for them to send me the information on it. They send me a sheet indicating the amount of money I contributed to the extended medical benefits in 2023.
I am unsure where to put this new sheet of information in Ufile as there is NO label on it like T4 or T4A. The sheet only states " the following benefits were paid by this employee..."
So where should I put this?
