Hello yoyoto,

You can claim the Canada training credit when you complete your income tax and benefit return.   There is no specific form.  You must complete
  Schedule 11, Federal Tuition, Education, and Textbook Amounts and Canada Training Credit
In UFIle :
  >> Tuition, education and textbook amounts (T2202,TL11A,TL11C)
  >>If eleigible it appears on line 45350 Canada training credit (CTC)

Hello taximan27,

*You were a resident in Canada throughout the year

*Tuition or other fees were paid to an eligible educational institution for courses that you took in the year that you are claiming the credit for or to certain bodies for an occupational, trade or professional examination

*The tuition and fees are otherwise eligible for the existing tuition tax credit

*You were at least 26 years old and less than 66 years old at the end of the year

Ontario Jobs Training Credit  is applied at same time as Canada training credit (CTC)

 T1 – Schedule 11 :

Enter this amount on  line 45350  of your return  Line 6

 Ontario – ON479 :

Ontario jobs training tax credit Amount from line 32000 of your federal Line 63055
